Credit/Debit Card on File

We have implemented a policy requiring a credit card held on file effective 5/1/2019.

Cancellation Fee

Your appointment is solely reserved for you only and canceling it less than 24 hours or not showing is not fair to other patients and the practice. There is a cancellation fee of $50 for cancellations less than 24-hour notice and missed appointments. This fee is not paid by insurance companies and will be charged on the card on file. This fee may be waived for emergencies if able to prove that appointment was not canceled in time or missed due to emergencies because life happens.

Disability Forms

Reform Psychiatry LLC/Jemima Mensah APRN, will complete FMLA forms for established patients only. NO Short term or long-term disability forms or FMLA will be completed for new patients on the first visit.

EMERGENCIES

  1. All emergencies should call 911 or go to the nearest emergency room after hours. Reform Psychiatry LLC is an outpatient psychiatric/mental health office and provider is available to see patients only during business hours.
  2. All attempts shall be made to see patients in crisis as soon as possible the same day, however, Reform Psychiatry is not a crisis center, or the nearest emergency room should always be the first action if a patient is a danger to self or others.
  3. The following resources can also be used after hours but calling 911 should be the first action if the safety of the patient and others is at risk.
